Donor Adoption Program Terms and Conditions
Program Fee Overview
Our goal is to help each patient achieve pregnancy. To ensure the highest standards of care and efficiency in our highly successful and FDA-compliant donor embryo program, we have implemented several well-considered protocols and guidelines.
The Donor Adoption Program Fee of $16,500 includes the typical number of office visits, staff time, overhead, ultrasounds, supplies, blood work, and specialized embryology laboratory procedures required during a standard Donor FET cycle. It also covers significant expenses incurred before the recipients’ adoption of the donated embryos/oocytes. All procedures adhere strictly to the comprehensive FDA guidelines for donated embryos/oocytes.
Clinical and Obstetrical Services
Fertility Treatment Center (FTC) provides clinical care and obstetrical services following the Donor Embryo FET cycle until a serum pregnancy test is performed. At this point, or when you are referred to an OB/GYN for continued obstetrical care, the services included in your package are considered complete. FTC provides pregnancy ultrasounds as requested by your provider, up to 12 weeks of gestation, which are also covered under this package. Whether or not conception occurs following the FET, the package concludes after your last serum pregnancy test or OB ultrasound.
Restrictions and FAQ’s
- Availability of Donated Embryos: The pool of available donated embryos is continuously updated, and we offer them on a first-come, first-served basis to ensure fairness and prompt service. This dynamic approach helps us manage the large number of participants efficiently while minimizing waiting periods.
- Program Fee: Our program fee is thoughtfully structured and non-negotiable, covering all necessary services for donated embryo adoption and Frozen Embryo Transfer (FET) without unexpected additional costs, providing you with financial clarity and peace of mind.
- Reservation and Fees: The program fee applies to the reservation (adoption) of one donor embryo set, with fees due at the time of selection. While you may purchase multiple sets, the full fee is required for each set.
- Single FET: Each adopted donor embryo set is used for a single FET. If only one embryo from your set is transferred during the first FET and additional embryos remain, additional FET fees and annual storage fees will apply. Our program fee package includes up to one year of cryopreservation, with subsequent storage fees due annually, if applicable.
Embryos
Utilization Within One Year: Your adopted donor embryo set(s) must be utilized within one year from the date of reservation. The Program fee covers comprehensive storage, 24/7 monitoring, security, and maintenance of your reserved frozen donor embryos. If you are unable to complete an FET within one year, ownership of the donor embryos will revert to our program, and you will receive an 80% Program fee refund.
Embryo Thaw Issues: In the rare event of an embryo thaw issue on the day of your FET, resulting in cancellation of your embryo transfer, we will promptly reschedule you for the next available date/time, with no additional fees or refunds necessary.
Cancelled FET Cycles: Should your FET cycle be cancelled due to low endometrial response or other uncontrollable factors, you may pursue an additional FET cycle. A discounted fee of $1,200 per FET cycle covers continued cryopreservation, additional monitoring visits, lab tests, ultrasounds, supplies, and the preparation involved in the FET.
No Satisfactory Embryos: If the embryo thaw results in no satisfactory embryos for transfer, you will receive a 50% program fee refund or a credit on your account for future services, reflecting our commitment to fairness and transparency regarding incurred expenses.
Oocytes
Utilization Within One Year: Your adopted donor oocyte set(s) must be utilized within one year from the date of reservation. The Program fee covers comprehensive storage, 24/7 monitoring, security, and maintenance of your reserved frozen donor oocytes. If you are unable to complete an FET within one year, ownership of the donor oocytes will revert to our program, and you will receive an 80% Program fee refund.
No Satisfactory Oocytes: If the oocyte thaw results in no satisfactory oocytes for insemination for embryo creation and subsequent transfer, you will receive a 50% program fee refund or a credit on your account for future services, reflecting our commitment to fairness and transparency regarding incurred expenses.
Negative Pregnancy Test or Miscarriage: In the unfortunate event of a negative pregnancy test or miscarriage with no remaining adopted donor embryos in your set, you may choose to reserve another donor embryo set (with the normal package fee due upon reservation) or explore other treatment alternatives with your provider.
Services Included in Program Fee
• Donation screening – There are extensive screenings, tests, analysis, and FDA compliance audits prior to donating embryos.
• Pre-adoption embryo cryopreservation, maintenance of embryos, and 1-year following adoption.
• Legal forms and documentation to confirm transfer of ownership (adoption) from our anonymous donor program to recipient.
• Waiver of any past-due donor embryo storage fees.
• Office visit to plan the FET cycle.
• Telephone advice calls with coordinator or provider, or in-person visits for adjustments of medications, etc.
• Examination & uterine size measurement.
• Office visits and vaginal ultrasounds to monitor ovarian stimulation, as necessary.
• Estradiol & Progesterone in-house lab tests.
• Venipuncture for blood collection.
• Endometrial disruption, if needed.
• Examination of the uterine cavity (HSG and/or SHG), if needed.
• Embryo preparation and thaw.
• Embryo transfer of cryopreserved embryos with ultrasound guidance.
• Assisted embryo hatching, if needed.
• Procedure room (i.e., surgery suite, facility fee, etc.) for embryo transfer and supplies.
• Post FET hCG pregnancy lab tests.
• Pregnancy related ultrasounds, as needed.
• Summary consult with Dr. Craig after FET to review cycle and plan next step, if needed.
Services NOT Included in Program Fee
• Medications – Pharmaceutical products and embryo glue (patient responsible to purchase separately)
• Additional Laboratory tests outside normal FET
• Hysteroscopy or other surgery
• Psychological Consultation, screening, review or counseling
• Embryo biopsy for PGT (includes embryo thaw, extended culture, biopsy, and re-freezing)
