We encourage both patient & partner to have blood drawn for FDA laboratory testing, performed for free whether blood is drawn at Fertility Treatment Center (no charge) or any LabCorp close to home.
Should you choose to have your physical examinations and blood testing carried out at Fertility Treatment Center, please call our office and speak with Michella or Marissa at (602) 975-0114 or complete the contact form below.
Upon receiving the completed embryo donation packet, we will promptly send you a $400 reimbursement check to the address noted on your donation consent form, and waive any outstanding or overdue storage fees.
